NOTES: Long Road home plans to have a legislative forum in August or September. We are trying to put together questions to use on a survey. If you can think of any please email Cheri no later than May 5th, 2006. Please note that the questions should be informative to those who are being surveyed as well as wanting to know where they stand on certain issues. Example if you want to ask a question about the ICWP you may want to describe the ICWP and how important it is and any problems then ask the question on where do you stand. We want the survey to do four things we want to bring attention to disability issues, inform the people being surveyed, find out where the person stands on the issue and educate the voters.
They need all kind of questions. Examples of topics: Medicaid waivers, consumer direction, money follows the person, life and death issues, nursing transition, affordable accessible housing, inclusion, visitability, education (disability related please), and accessible transportation.
Please share as widely as possible
